Police rail unit Sgt George Tilbury said the carer tried desperately to lift Ms Gath to safety before being forced to jump clear to save her own life.

The pair were on their way to a rehabilitation appointment for Ms Gath in Shenton Park when the accident happened about 11.20am.

Sgt Tilbury said the driver used an emergency brake, sounded the horn and flashed headlights.

Train passenger Bill Taylor said the driver sounded his horn “long and loud” before he heard “one hell of a bang”.

“We hit something, it was something metallic, something hard,” he said.

Public Transport Authority spokesman David Hynes said Carlisle train station was equipped with audible and visible warning devices that met Australian standards.
